We do not confirm benefits at Maple Valley Acupuncture. Here are some questions to ask your insurance company:

We do accept car accident/PIP claims. Please fill out this form prior to your first appointment. We only bill open PIP claims with available funds.
What codes are billed?
First Treatment
99202 ($90) or 99203 ($175) - office visit code, code depends on how long the intake process is
97810 ($60) acupuncture first 15 minutes OR 97813 ($70) acupuncture first 15 minutes with electrical stimulation
97811 ($45) acupuncture additional 15 minutes OR 97814 ($57) acupuncture additional 15 minutes with electrical stimulation (two of these codes will most likely be billed, but depends on overall treatment time)

97026 ($10) TDP heat lamp
97140 ($20) Manual therapy - running cupping or gua sha

Return Treatment
99212 ($88) or 99213 ($142) - office visit code for re-evaulation. Billed every 5-8 visits, if a new chief complaint has come up, or if patient hasn't had a treatment in 60+ days
97810 ($60) acupuncture first 15 minutes OR 97813 ($70) acupuncture first 15 minutes with electrical stimulation
97811 ($45) acupuncture additional 15 minutes OR 97814 ($57) acupuncture additional 15 minutes with electrical stimulation (two of these codes will most likely be billed, but depends on overall treatment time)
