Welcome new patients! If you fill out your paperwork (below, or online in your Jane account) prior to your appointment, there is no need to arrive early. If you would like us to bill your insurance company, please bring your insurance card.

Please wear loose, comfortable clothing. Try to have a meal or snack before your appointment and limit caffeine consumption. Call our office at 425-413-3801 if you have any questions about your appointment - we are happy to help make your experience as comfortable as possible.
